CPT Group, Inc. its subsidiaries and affiliates (“CPT Group”) provides the information and services on this website (“Site”) under the following Terms and Conditions, which govern the information you provide to us and your use of this Site. 2. Functioning of the site

2.a.  No Warranties
No representations or warranties are given that any functions contained in the Site will be uninterrupted, error-free, virus-free, or free from harmful components, or that any defects will be corrected. Furthermore, no transmission over the Internet can be guaranteed to be secure.

2.b.  Access to Protected Areas of the Site

Access to and use of password protected and/or secure areas of the Site is restricted to authorized users only. Users agree not to share user names or password information needed to gain access to such areas. Any prospective users are required to register and receive their own unique user names and passwords before attempting to gain access to protected areas of the Site. Unauthorized access to such areas is prohibited and may lead to criminal prosecution.

2.c.  Agreement to Receive Notices Electronically
Users agree to receive all notices, including but not limited to notices of claims deficiencies, through the posting of such information to the Site, e-mail sent to users’ registered e-mail addresses, or other electronic means rather than by mail, at the sole discretion of CPT GROUP.

3.a.  Posted Information
Although CPT GROUP seeks to provide accurate and updated information on this Site, we cannot assure that all posted information is current and accurate. Accordingly, the Contents of this site are provided “as is” and without any warranties of any kind whether express or implied, including but not limited to the implied warranties of merchantability or fitness for a particular purpose. We make no representations about the accuracy, reliability, completeness, or timeliness of the Contents of the Site. Claims processing is subject to ongoing quality assurance and final reporting.

3.b.  Uploaded Claims Information
CPT GROUP is not responsible for the accuracy of data uploaded to this Site by users, or for delays caused by users’ failure to follow uploading, formatting, or other instructions. Moreover, CPT GROUP is not responsible for the unauthorized submission or uploading of information where users have intentionally or negligently provided unauthorized individuals with access to user names or passwords needed to access protected areas of the Site.

 4. Payment of uploaded claims

Submission of claims information does not guarantee payment of a claim. All claims information is subject to processing and validation, and all claims are subject to subsequent determinations of eligibility, calculation and approval. Furthermore, all claims must be accompanied by a signed claim form and appropriate supporting documentation, which may be uploaded as .pdf files (preferred method) or submitted by mail.

 5.  No professional advice

The information provided on the Site is distributed with the understanding that CPT GROUP is not rendering professional advice of any sort. The Contents of this Site cannot replace or substitute for the services of trained professionals in any field. These Terms and Conditions, which shall be deemed to include CPT GROUP’s Privacy Policy (available at https://www.cptgroup.com ), constitute the entire agreement between you and CPT GROUP pertaining to this Site and supersede any and all other agreements, whether oral or in writing. The failure of CPT GROUP to insist upon strict compliance with any of these Terms and Conditions shall not be construed as a waiver with regard to any subsequent failure to comply with such term or condition. User agrees that any proceedings relating to this Site shall be brought in courts located within the State of California, consents to the jurisdiction of such courts in connection with any such proceedings and acknowledges that any breach of these terms and conditions by user shall cause irreparable harm to CPT GROUP sufficient to warrant the granting of equitable relief.
